The Western New York Care Coordination Program (WNYCCP) is a unique six county consortium dedicated to transforming services for adults with severe behavioral health conditions.

"The integration of person-centered practices means a fundamental shift in goals for individuals. The goal is no longer simply stability, but recovery."

The participating upstate New York counties (Chautauqua, Erie, Genesee, Monroe, Onondaga, and Wyoming) are currently serving nearly 3000 such persons through 12 not-for-profit care coordination agencies.

In 2009, WNYCCP expanded its efforts to develop managed local/regional systems of care for delivery of behavioral health services and coordination of physical and behavioral health services for people with serious mental illness by contracting with Beacon Health Strategies, LLC.

About Person-Centered Planning

Person-Centered Planning Recognized as Emerging "Best Practice" by SAMHSA The Substance Abuse and Mental Health Services Administration (SAMHSA) held a consensus group to address person/family centered planning (i.e. adults, children, and families).
