Physical and Behavioral Health Integration: Physical Health Survey
The Western New York Care Coordination Program conducted a physical health survey of enrolled individuals across its six-county region in order to understand the types of needs enrollees have, the types of physical health services enrollees currently receive, and the types of physical health services enrollees would like to receive in the future. Each care coordinator was asked to provide a copy of the questionnaire to individuals they worked with and return the responses to WNYCCP. A total of 639 surveys were returned and summarized for analysis. Link to Report

The exact survey was used again and each care coordinator was asked to provide a copy of the questionnaire to individuals they worked with and return the responses to WNYCCP. A total of 1115 surveys were returned and summarized for analysis. Link to Report
